What to Bring
Please have the following ready before your appointment: your doctor’s lab order or requisition form, a valid government-issued photo ID, and your insurance card (if applicable). Appointments typically take 10 to 20 minutes depending on the number of tests ordered.
What to Expect
- Schedule online — pick your date, time, and location via our booking page
- We arrive at your location — fully equipped, on time, and ready to serve
- Fingerprinting completed — careful, clean, professional ink prints on FD-258 cards
- Cards in hand — you receive your completed cards immediately to submit to your agency
ID Required: Please bring one valid government-issued photo ID — Driver’s License, State ID, Passport, or Military ID. Have any employer-provided paperwork ready as well.
Appointment duration: 10–15 minutes per person.
